Big Doc got the call for Wednesday's friendly against USA after Sunderland's Niall Quinn pulled out with a neck injury.
It's another huge boost for Gary after he finally made his Premiership comeback at the weekend after seven months out with a broken leg and ankle.
He was a regular in squads during Ireland's successful World Cup qualifying campaign and his versatility may still be enough for a place on the place for this summer's finals.
McCarthy said: "Gary was in the fold early on and it's not nice to get an injury at any stage, but particularly when you are involved in a World Cup campaign and there is an opportunity there, so to have it dashed by injury is not very nice.
"It will be difficult for him after his injury but he is part of my squad now, and whether he goes to Japan will be up for debate later on."
